Wstęp:
Jednostronna głuchota (SSD) charakteryzuje się występowaniem głębokiego lub całkowitego ubytku słuchu w jednym uchu przy normie słuchowej w drugim. Brak słyszenia binauralnego prowadzi do trudności w rozumieniu mowy, lokalizacji dźwięku oraz obniżenia jakości życia. Dostępne są różne metody leczenia SSD, dobierane indywidualnie w zależności od: etiologii, czasu trwania zaburzenia, uwarunkowań anatomicznych, wyników badań audiologicznych i potrzeb pacjenta. Najczęściej stosowaną metodą leczenia jest implantacja ślimakowa. Inną dostępną opcją jest zastosowanie implantów wykorzystujących przewodnictwo kostne. Celem pracy była ocena efektywności klinicznej i bezpieczeństwa zastosowania aktywnego implantu kostnego Sentio w konfiguracji CROS u pacjentki z SSD.
Opis przypadku:
Przedstawiono przypadek 50-letniej pacjentki z jednostronną głuchotą spowodowaną nagłą głuchotą z uszkodzeniem części przedsionkowej błędnika. Ze względu na całkowitą ossyfikację ślimaka niemożliwe było wykonanie implantacji ślimakowej. Pacjentka została zakwalifikowana do wszczepienia aktywnego implantu wykorzystującego przewodnictwo kostne Sentio w konfiguracji CROS. Zabieg i okres pooperacyjny przebiegły bez komplikacji. W dniu aktywacji procesora oraz podczas wizyt kontrolnych (po miesiącu oraz po sześciu miesiącach) przeprowadzono badania rozumienia mowy w szumie oraz ocenę trudności słuchowych w codziennych sytuacjach za pomocą kwestionariusza APHAB. Po implantacji zaobserwowano wyraźną poprawę rozumienia mowy w szumie, zarówno podczas aktywacji procesora, jak i kolejnych wizyt kontrolnych. Pacjentka zgłaszała także zmniejszenie odczuwanych trudności słuchowych.
Wnioski:
Uzyskane wyniki potwierdzają skuteczność kliniczną oraz bezpieczeństwo zastosowania implantu Sentio w konfiguracji CROS w opisywanym przypadku. Obserwacje te wymagają dalszej weryfikacji w badaniach obejmujących większe grupy pacjentów.
