Wstęp:
Implantacja ślimakowa u dzieci z wrodzonym, obustronnym głębokim niedosłuchem czuciowo-nerwowym stanowi podstawową interwencję wspomagającą rozwój percepcji słuchowej oraz kompetencji językowych. Wczesne wszczepienie implantu pozwala na optymalne wykorzystanie okresu największej neuroplastyczności mózgu i uzyskanie potencjalnie lepszych wyników w rozwoju słuchowym i językowym, jednak wymaga uwzględnienia ryzyka związanego z procedurą chirurgiczną oraz długoterminowym funkcjonowaniem urządzenia.
Materiał i metody:
Narracyjny przegląd literatury przeprowadzono z wykorzystaniem elektronicznej bazy PubMed. Strategia wyszukiwania opierała się na anglojęzycznych słowach kluczowych w formie kombinacji zgodnych z systemem Medical Subject Headings (MeSH). Do analizy włączono wyłącznie artykuły opublikowane po 2000 roku, w których badano wpływ wczesnej implantacji ślimakowej na rozwój umiejętności słuchowych i językowych u dzieci.
Wyniki:
Do analizy zakwalifikowano 23 pełnotekstowe artykuły, w których opisane badania potwierdziły korzystny wpływ wczesnej implantacji ślimakowej na rozwój słuchowy i językowy u dzieci. Najwyższe wyniki obserwowano u dzieci implantowanych przed 12. miesiącem życia, zarówno w zakresie funkcji słuchowych, percepcji dźwięków, jak i rozwoju językowego. Analiza literatury pozwoliła wyróżnić cztery główne obszary funkcjonalne: 1) funkcje słuchowe i percepcja dźwięków; 2) nadawanie (produkcja) i zrozumiałość mowy; 3) rozumienie języka i zasób słownictwa; 4) rozwój komunikacyjny i społeczny. Dane dotyczące bezpieczeństwa wskazują, że implantacja u niemowląt jest procedurą stosunkowo bezpieczną, z niską częstością poważnych zdarzeń niepożądanych.
Wnioski:
Wczesna implantacja ślimakowa wspomaga rozwój słuchowy i językowy u dzieci z prelingwalną głuchotą, przy czym największe korzyści obserwuje się u dzieci implantowanych przed 12. miesiącem życia. Uzyskane wyniki były również modulowane przez czas użytkowania implantu, zastosowane narzędzia oceny oraz czynniki kliniczne i środowiskowe.

Niniejsze badania i artykuł nie otrzymały żadnego specjalnego grantu od agencji finansujących w sektorze publicznym, komercyjnym ani non-profit.
